Please fill out the attached permission to include with your final payment. Please check your TRP (travel reward points) statement and fill out the final amount accordingly.

School Board Waiver Form:

The Regina Public School Board requires all parents to fill out the attached waiver form.  Please ensure this is included in the envelope

Medical Form:

If you have not completed a medical form this year, please fill out the attached form.

To improve communication and assist with music group activities, we are looking for parents who would:

 Be a central point of contact to the music group’s parents for teachers and CAMPA

 Collect forms and fees from students in group (during school hours) and provide general record keeping of form results and total fees collected

 Contact group parents/guardians when needed (information distribution, gathering forms/fees)

 Organize group social activities

Please provide your name, contact phone number, student’s name and groups they are in. We would like to have group reps for all bands and choral groups in place for next semester.

Note from Maureen Pennington:
Hi CAMPA families
As you may know this is my last year as Campa's bingo co-ordinator as my daughter graduates this year.  We are looking for someone to take over for me.  I would like to work with this person over the next couple of months to ensure they know what they are doing, if interested contact me directly at mpennington@sasktel.net

Here are the job responsibilities:

1:  Be at the bingo hall by 4pm to count paper, get counters ready, fill aprons, get floats ready, check nevada tickets.
2:  Inform CAMPA families the dates you need workers, keep track and send out reminders before the bingo.
3:  Attend bingo meetings, usually 2 times a year.
4:  Attend board meetings monthly and give bingo report.
5:  Keep track of workers and give to administrator or treasurer to give out TRP's.
6:  Assign workers to jobs once at hall.
7:  Work counter and/or cash and balance out at end of night.
8:  Give papers to adminstrator after bingo, keep copy for your records.
9:  Work with adminstrator and bingo hall to let know of any dates CAMPA will not be available.
10:  Ensure workers are spread out around hall and not grouping together.
The bingo co-ordinator gets $50.00 in TRP's.

The Glass Angel Gift set are sold for  $10.00  and you will receive $2.00 in TRPs.

Delivery week of Nov 23rd

Every student which sells their first set will have their name entered in  a draw for a basket of Avon products.

Featuring: The  Junior Voices, Senior Voices, and the Chamber Choir Tuesday, October 27^th at 7:00 p.m. in the Currie Auditorium.  Admission By Donation.

Featuring:  The Grade 9 Band, Concert Winds and Wind Ensemble Wednesday, October 28^th at 7:00 p.m. in the Currie Auditorium. Admission By Donation.

All Campbell Staff and Students are encouraged to attend and support our Campbell Music Students!!!  Bring your family and friends!!!!

This CAMPA fundraising initiative runs from Sept 28th - Oct 16th.

If ordering online, Campaign Code is SK378  and student must hand in copy of confirmation order

Or hand in payment with completed order form in white envelope with students name on it to the Campbell Music Office.
